At a glance: How to approach activity, strength and flare-ups when lower back pain has become persistent.

Movement is often part of recovery

For many people with non-specific lower back pain, staying active and gradually rebuilding capacity is more useful than prolonged bed rest. The right approach depends on symptoms and clinical assessment.

Find a tolerable starting point

Short walks, comfortable mobility work and simple strengthening movements can be adjusted to what you can currently manage. Progress slowly, focusing on function rather than a perfect posture.

Build strength progressively

A tailored programme might include hip hinges, squats, carries or trunk endurance exercises, beginning with suitable ranges and loads. No single exercise is essential for everyone.

Respond to flare-ups thoughtfully

Temporary symptom fluctuations do not always mean harm, but significant or worsening symptoms should prompt reassessment. Adjust intensity and volume rather than automatically stopping all movement.

Recognise warning signs

New bladder or bowel dysfunction, numbness around the groin or saddle area, or progressive leg weakness requires urgent medical assessment. Persistent or unexplained symptoms also deserve professional review.
